If you return to the Netherlands after a period abroad, you are required to take out Dutch health insurance within four months of re-registering in the Personal Records Database (BRP). In practice, this means: as soon as you are subject to the insurance obligation again, you must take out a policy as quickly as possible. This article answers the most frequently asked questions about health insurance upon return, so that you enter the Netherlands well prepared.
From what date are you required to take out insurance again?
You are required to take out Dutch health insurance from the day you become subject to the insurance obligation again. This is generally the day on which you register in the BRP of your new municipality. If you have already arranged your registration, the insurance obligation takes effect immediately on that date.
Important: you officially have four months to take out health insurance after becoming subject to the insurance obligation. But do not postpone this for too long. If you register later, the insurer can make the coverage retroactive to the date of registration, but you run the risk of a period without coverage if something goes wrong. Take out your insurance preferably on the day of registration or as soon as possible afterwards.
Do you have a partner or children moving with you? They also need to register and take out insurance as soon as they live in the Netherlands. Children under 18 are covered free of charge under a parent’s policy.
Which health insurers accept returnees from abroad?
All Dutch health insurers are legally required to accept every person subject to the insurance obligation for the basic insurance, regardless of your health or how long you have lived abroad. This is called the acceptance obligation. You can therefore go to any health insurer, even if you have not lived in the Netherlands for years.
The acceptance obligation applies exclusively to the basic insurance. For supplementary insurance, an insurer may ask questions about your health and potentially refuse you or charge a higher premium. It is therefore advisable to arrange the basic insurance first upon return and then assess whether a supplementary policy is needed.
Compare insurers via the official government comparison site or via independent comparison sites. Pay attention not only to the premium, but also to the deductible, reimbursements for general practitioners and specialists, and the quality of customer service.
What do you need to apply for health insurance?
To apply for Dutch health insurance, you will in most cases need your Citizen Service Number (BSN), a valid proof of identity, and your Dutch bank account. You receive your BSN when registering in the BRP at the municipality. Without a BSN, you generally cannot take out insurance.
Make sure you have the following matters in order before submitting the application:
- BRP registration: This is the first step. Make an appointment at your new municipality as soon as you know when you will arrive.
- BSN: You receive this automatically upon registration if you did not already have one.
- Valid proof of identity: Passport or identity card.
- Dutch bank account: Required for direct debit of the premium. Open this in good time, if necessary already from abroad via a Dutch bank.
- Proof of cancellation of your foreign insurance: Not always required, but useful to have on hand.
The application itself is handled entirely online by most insurers and is completed within ten minutes.
How does the healthcare allowance work after returning to the Netherlands?
The healthcare allowance (zorgtoeslag) is a monthly government contribution to keep the health insurance premium affordable for people with a low or middle income. After returning to the Netherlands, you can apply for the healthcare allowance via the Tax and Customs Administration (Belastingdienst), as soon as you have taken out health insurance and are registered in the BRP.
Whether you are entitled to the healthcare allowance depends on your income and possibly the income of your allowance partner. In 2026, specific income thresholds apply that are determined annually by the government. Check via the website of the Belastingdienst whether you qualify and how high the allowance is based on your situation.
Apply for the healthcare allowance as soon as possible after your return. You can apply for the allowance retroactively for the current year, but do not wait too long: applications for previous years are subject to deadlines. The allowance is paid out monthly to your Dutch bank account.
What if you already incurred medical costs abroad shortly before returning?
Medical costs you incurred abroad before taking out your Dutch health insurance are in principle not covered by the Dutch basic insurance. Your Dutch health insurer only reimburses costs incurred after the policy has taken effect.
There are two situations where this may be different:
- You had foreign insurance: If you were insured abroad, submit the costs to that insurer. Make sure you properly document the termination of that policy and keep all invoices.
- You had no insurance abroad: In that case, the costs are at your own expense. The Dutch health insurance has no retroactive effect for costs incurred before the start date of the policy.
Did you start a medical treatment shortly before leaving the country that is being continued in the Netherlands? Discuss this directly with your new health insurer. Some treatments may be reimbursed if the medical necessity is demonstrated and the treatment takes place in the Netherlands.
When is it wise to also take out supplementary insurance?
Supplementary insurance is wise if you need care that is not or only partially reimbursed by the basic insurance, such as dental care for adults, physiotherapy, glasses and contact lenses, or alternative medicine. The more extensive your healthcare needs, the sooner a supplementary policy pays off.
After a period abroad, there may be specific situations where supplementary insurance has extra value:
- Dental backlog: If you received less dental care abroad, dental insurance can quickly pay for itself.
- Physiotherapy: After a move or with chronic complaints, physiotherapy may be necessary. The basic insurance only covers this to a limited extent.
- Travelling abroad: If you still regularly travel to your previous country of residence, supplementary travel insurance or coverage for foreign healthcare may be worthwhile.
- Children: For children, supplementary insurance for orthodontics or extra dental care is worth considering.
Bear in mind that insurers may request a health declaration for supplementary packages. Take out the supplementary insurance preferably at the same time as the basic insurance, so that you have to fill in as few health declarations as possible.

Veelgestelde vragen
